Changes to AF Form 354, civilian identification card

PETERSON AIR FORCE BASE, Colo. -- Effective April 1, active and retired Air Force civilians and their family members will no longer be able to access the base using the Air Force Form 354.

The AF Form 354 is an obsolete credential identifying Air Force civilians, retirees and their eligible family members.

Anyone attempting to enter the installation using an AF Form 354 on April 1 or after will be issued a 24 hour pass to allow them to obtain the proper base credential for access.

The AF Form 354 will no longer be issued because the card doesn't meet updated security requirements established by the Department of Homeland Security. Specifically, installation access now requires a secure card with identity vetting and online capabilities, such as bar code scanning by the Defense Biometric Identification System scanners located at all the installation gates. The AF Form 354 doesn't have a barcode, and thus cannot be scanned at the gates.

Family members of Air Force civilians (active and retired) who have a current AF Form 354 may go to the Civilian Personnel Section with a current copy of their sponsor's Standard Form 50 (Notice of Personnel Action) and complete the application for an identification card/DEERS Enrollment. Additionally, the sponsor should present their current Common Access Card at the time of application. In addition, family members of active and retired Air Force civilians who desire to make an initial request must provide supporting documents (i.e. marriage license, birth certificate, unexpired passport). The CPS will provide the applicant with a DD Form 1172-2, Application for Identification Card/DEERS Enrollment to take to the Visitor Control Center at the West Gate to be issued a DBIDS card.

Also, the Peterson VCC will no longer issue the Air Force Form 354. The VCC will only issue a DBIDS card to civilian dependents. The DBIDS card will have an expiration date that will match the civilian sponsor's ID card. This card will allow base access and MWR services. The card will identify the MWR services on the remarks section on the back. The information in the remarks section will state "CIVILIAN DEPENDENT MWR ACCESS AUTHORIZED." Retired civilians will be required to have a Defense Department Civilian Retiree ID Card or other credentials for installation access.
